In order to ensure the highest level of customer service, Toyota Material Handling sends surveys to all of our customers upon the purchase of equipment and/or a warranty claim. 

In order to change the name of the person who should receive these surveys, please complete and submit this form within 24 hours of your equipment delivery or completion of your warranty claim service.
